Claim process:

1. Download, print and complete this claim form.
2. Submit the completed claim form along with a copy of the invoice from an authorised reseller (must
be based in the UK and be from Misco, DSGi/Equanet, Insight or Dabs.com) to:
NETGEAR® Smart Switches Promotion, Dept 84405, PO Box 34, Tipperary, Ireland.
3. Upon receipt of the completed entry form and proof of purchase you will receive either a
confirmation e-mail or an e-mail to inform you we are missing specific data to process payment.
4. You will receive payment within 10 working days from the date of the confirmation e-mail.
5. A letter will also be sent to the accounts department of your indicated public sector department/
institution informing them of the payment and reason for payment.
